Instructions for Data Entry

Enter patient data in the format below, **separating each value with a comma**:

Format: Bilirubin, INR, Creatinine, Sodium, Albumin, Gender

Gender: Use male or female.

Dialysis Patients: If the patient is on dialysis, enter "4" as the creatinine value.

Example Data:

• 2.5, 1.2, 1.0, 135, 3.5, male (Male patient)
• 1.8, 1.0, 0.9, 140, 4.0, female (Female patient)
• 3.2, 1.5, 4.0, 138, 3.2, male (Dialysis patient - creatinine set to 4.0)

Note: Ensure that Bilirubin, Creatinine, and Albumin are in **mg/dL**, Sodium in **mEq/L**, and INR is **unitless**.
